As I have never come remotely close to one of these I wouldn't know. But I see people bragging about it all the time. How do they know?!?!

It’s a guesstimate 
They count from the time 4.2 went live until about 18 hours which is, many believe, the max spawn time.
When I tamed Deathmaw, it was just one hour after 4.0.6 went live, so I’m guessing that was “server first”.

You get an achievement called “If You're Not First, You're Last”, the pet is granted two extra talent points, then Blizzard mails you a full set of top-tier PvE and PvP epics and a limited edition flying Loque'nahak mount with a laser cannon mounted on his head.
Holy crap… you didn't get one did you..?!
Have you checked your mail?!
If you got a realm first, grats..!!!
