I've always felt this to be true, but I'm a useless PvPer anyway. So I've been watching some duels in the Dalaran Sewer arena. Funny… I don't see many hunters down there! Lots of players approach and challenge me, though.
The only times I've seen a hunter win a duel with a more or less equally geared spell caster it has been a BM hunter with a BW/RF burst, hitting so hard and fast at the start of the duel that the other guy – maybe taking his eye off the ball after his first near-killer spell – does not seem to have time to react and goes down quickly. Whatever happens, it's over in seconds.
Many other duels I've watched, between other classes with good self-healing abilities, go on for ages. I recently watched a druid apparently playing with an equally-geared pally for around 20 minutes, before he got bored and stopped healing himself (so he told me afterwards, and it appeared to be true). At this point the pally's spell and melee damage overwhelmed his own and he submitted fairly quickly. I can't see that there is any way a hunter of any spec could survive so long, without the same level of self healing.
Maybe it's by design that hunters are only effective in PvP as team players?
But what do I know? Nothing really – only what I get hit with in a one-on-one in WG, and what I see of duels. Hopefully our PvP king will put me right on this, and tell me about all the lovely hunter survival tactics that I'm overlooking and still need to learn about!

I think it's outstanding. The initial heal hits me for around 2.8k, then there are 5 subsequent ticks for 1.5K+ each over 10 secs, making the total heal well over 10K. I have it macro'd so that I can use it preemptively, as the auto-cast only kicks in at <50%. I find it works best if you activate it around % or so, if you know you're about to take some heavy damage.
